Welcome New Members

I welcome you to our team and we look forward to working with you through the completion of the application process. To assist you in this process you have been provided access to our member only area (where you’re reading this)

Two items which will be helpful right away are the calendar (including team mtg dates and on call months and directions to the FTC where our meetings are held. You are welcome to attend meetings at any time during the application process.

There are several steps in the application process that must be completed before you are deemed deployable.
During the E-Induction phase you will be sent and must complete a governement travel credit card application to be used for deployments ONLY.


1) The first step which you have completed is the application.

Upon our receipt of your application we then submit your name for the finger print and photo appointment. You will be then contacted by the Federal office who will schedule an appointment with you.   Please send me (Kim)  an email when you have confirmed the appointment and again upon completion. . This allows us to follow up and forward appropriate documents to NDMS Headquarters on your behalf.


2) The next step is the EQIP process which is a federal background check. This process is initiated via online and  is between you and the EQIP staff.   (we are usually copied) Should a problem arise it is important that you communicate with the your EQIP contact that is assigned to process your file. From past experience we have found them more than willing to work with you to answer any questions or assist if an access problem arises. I encourage you to make notes of the info you provided since you may be interviewed and you will want to remember what you have documented.  I would also suggest you notify people that you have listed as references as they will be contacted.


3) Once you have cleared the EQIP process your Human Resources file will be established. You will receive an “offer” called an "E Induction" for the position for which you applied. There is a position "acceptance" letter which you must complete. On the letter will be an eligibility date. Once you have signed this and the date is reached you are eligible to be available for deployment.


4) There will also be a Credential Smart process. This is a process to verify your credentials (licenses, certs., etc) Upon sucessful completion of steps 1,2,3 above, Credential Smart will activate a file for you.  We will then send an email to you with your Credential Smart access information. This is a system to maintain and verify your licenses and certificates. Please be reminded to send a hard copy of any license/certification renewals that you may complete. This information should be sent to dmatny4@aol.com.  You may scan or fax these documents.


Once you are a deployable member you will be issues passwords to gain access to a few internet areas that are adminsitered by NDMS. These are important areas to become familar with.  The external and internal NY4 websites list links to them.  On the external site (www.dmatny4.com)  they are listed under links. On this site they are listed under "reference". I have provided an overview of these NDMS internet areas below:


Responder E-Learn - This site provided official online training sessions for DMAT personnel.  These sessions are valuable in providing information for future responses. All members are required to participate in these sessions and should check back intermittently as sessions are added. To access this area use the team web site member area under training. Directions and links are provided.  The system is updated once a month with new member passwords. This may take several weeks to have you initiated.


My Pay – This is an important area where you can review any payments you are scheduled to receive. It also provides access to information such as W-2’s and your current pay grade.


Teams.hhs.gov  This is a federally maintained site where information may be found. Please be reminded any information changes have to be sent to our AO Desiree’ so it can be updated in all system. This site proved invaluable for providing information on the Haiti responses. 

Gov trip – This site provides some travel information during deployment and for travel worksheets 

JP Morgan Chase Govt Credit Card –  This site provides information on the status of your HHS credit card account.
